Orthognathic surgery or Corrective Jaw Surgery combines maxillofacial surgery or jaw surgery with orthodontics. The term “orthognathic surgery” is latin derivation of the word "Ortho" to strighten and "Gnathic" meaning the jaw.The ideals of this type of surgery are to improve one’s bite, creat a balanced facial apperance and improve breathing during sleep. The surgeries done are designed to correct misalignment of the jaw and make the jaw fit together properly. Facial imbalances such as an underbite, overbite or dentofacial deformity can be corrected using orthognathic surgery. Orthodontics are used to correct malocclusions or crowding of the teeth and to improve smiles lines before surgicalk movemnts of the jaws. TMJ disease can be corrected or prevented using orthognathic jaw surgery. Patients who have jaw problems that can’t be treated with orthodontics alone have orthognathic surgery to correct the jaw abnormality so the teeth work properly.

Here are some indications to why one might need to have corrective jaw surgery;

  • difficulty chewing, or biting food (anterior open bite)
  • difficulty swallowing
  • chronic jaw or jaw joint (TMJ) pain and headache which will lead to bone melting in the TMJ
  • excessive wear of the teeth
  • open bite (space between the upper and lower teeth when the mouth is closed)
  • unbalanced facial appearance from the front, or side
  • trumatic facial injury such as jaw fractures
  • birth defects as as Pierre Roban Syndrome, Hemifacial Microsomia or Treacher Collins 
  • receding chin as in retrognathia or microgenia 
  • protruding jaw in overbite, mandibular hyperplasia
  • inability to make the lips meet without straining vertical
  • chronic mouth breathing and dry mouth in patients with breathing difficultis, open bite and gummy smiles
  • obstructive sleep apnea (breathing problems when sleeping, including snoring)

Surgery on the upper jaw is called a maxillary LeFort I osteotomy and surgery on the lower jaw is called a bilateral sagittal split mandibular osteotomy. A patient can have one or both of these surgeries at the same time in order to correct the alignment of the upper and lower jaw and to improve the functionality of the upper and lower jaw. Occasinaly the chin bone, sliding genioplasty is also moved for optimal facial aesthetics and balance. The surgery is generally done using incisions inside the mouth so that there is no external scarring visible. Small titanium plates and screws are used to hold the bones together till the bones heal and fuse.
